  1. Table shows sample demographics obtained from the qualitative phase (semistructured interviews) and quantitative phase (online survey). Significant differences were found in years of practice and academic affiliation depending on country in the quantitative phase sample (p < 0.05): 3–10 years (13% in Germany, 24% in the United Kingdom, 26% in Japan, 35% in the United States), 11–20 years (70% in Germany, 45% in the United Kingdom, 44% in Japan, 46% in the United States), 21 years or more (17% in Germany, 31% in the United Kingdom, 30% in Japan, 20% in the United States), and academic affiliation (56% in Germany, 83% in the United Kingdom, 55% in Japan and 28% in the United States).
